The form ACT 39 (Transfer or Retirement of Equipment or Supplies) is not available online. It is available for sale at Central Stores (item #72890033).

The Commercial (Custom) Invoice Form is required for all International Federal Express and International UPS packages except for letters or other documents. Print and complete this form.

Use this form to request assistance moving an office or moving your entire department. Also use this form when you need to move items from your location to another location. If possible, please request at least 10 working days before your move.

Do not use this form when putting items in Surplus. See Surplus.

Use this form to request pickup of the following items:

  • Packages ready for shipping
  • Recycling of printer and fax toner cartridges
  • Returns to vendor
  • Returns to Central Stores

Use this form to reserve storage space and to request pickup of your items for storage at Central Stores.

Use this form to request rental of tables and chairs for University conferences and seminars. Please request at least 10 working days before the event.
